Character Sheet

Red Asterisks (*) signify a required field

*Name: (Your character's name and/or aliases. List nicknames in quotation marks.)
*Gender: (Self-explanatory, I hope.)
*Race: (State your character's race, whether human or vampire or anything else. Whether you say stuff like "Black" is up to you.)
*RP/CYOA of origin: (Which RP or CYOA does your character originate from?)
*Card Character?: (Whether or not the character takes the form of a card. If yes, name who holds the card after a "~" (ex. Yes ~ Bob))
*Age: (How old is your character? If their age doesn't match their appearance, feel free to also state how old they look; ex: 1,000 yr. old girl who looks 10)
*Appearance: (Describe your character's appearance here. You may also link to other images to help explain.)
Card Description (if applicable): (While in card form, what is printed on their card? Describe the image/pose the character is taking in their illustration. Note that Character Cards typically have a blue border.)

Character Relationships:
(You can list the relationships between this character and another character here (preferably characters in the crossover, but can include characters particularly important to their story).
